It is mandatory to fill up Emergency Certificate Application form online at the following link: (https://mportal.passportindia.gov.in/mission/). It is also mandatory to upload photograph (click here for photo specification), signature and supporting documents on the website online to complete the application form. 

Check-List of Documents:- 

  1. Application form as mentioned above 
  2. Self-attested copy of current Indian passport (Front and last page with personal particulars)
  3. Original Police report for lost passport clearly mentioning the passport number 4. Copy of booked return flight ticket
  4. Copy of the Visa/Resident Permit
  5. Copy of City Registration (Meldenbischingung), if available. 
  6. Receipt/ proof of the fee payment if paid through bank transfer OR one may also pay in cash at the counter
  7. Attach a self-addressed prepaid stamped (Euro 4.50) envelope of appropriate size for return of serviced documents. 
  8. Self Declaration/ Undertaking clearly giving the reasons for applying Emergency Certificate. 
  9. One photograph of size 3.5 cm by 4.5 cm (same as the one uploaded on the website)

SUBMISSION OF APPLICATION 

Application along with above mentioned supporting documents are to be submitted at the Consulate personally by scheduling an appointment by writing an email at cons[dot]munich[at]mea[dot]gov[dot]in. Applicant (s) presence is mandatory for Emergency Certificate, as he/she has to sign in front of the Consular Officer. 

Processing Time: 03 working days from the date of receipt of application in Consulate.
